The applicant shall apply for registration using the proforma letter of application supported with relevant information/materials. The proforma letter of application and the list of information/material to be submitted can be found in Annex 3 [PDF/DOC] and 4 in the [Scheme Document] .
Note: The company’s chop shall be stamped on the Proforma Letter of Application and all the document front covers provided by hand, or through post, electronic mail, or e-application channel to the Water Supplies Department. All photocopy test reports submitted to the Department shall be certified as true copies issued by the testing laboratory appointed by the participant. Upon the request of the Department, the participant is required to provide the original copy of the test reports/certificates.
If the application is accepted, the participant will be notified of the result within 20 working days upon the receipt of all necessary information requested. Soft copies of the Label will be transmitted by the Department to successful applicants soon after the approval of the respective application. The participant shall then affix/print the Labels correctly to the registered drinking water dispensers and filters.
