  • Form WWO 46 (Ver 1/2018) was split into Form WWO 46 (ver. 01/19) and Form WWO 1149.  The Form WWO 46 (01/19) and the Form WWO 1149 shall be used on or after 1 January 2019 in all application with Part I & II of Form WWO 46 not yet submitted and submission of the old version of Form WWO 46 (1/2018) Part I & II will not be accepted on or after the same date. For details, please refer to WSD Circular Letter No. 6/2018.
